Community Services provides Medicaid Service Coordination for Individuals who live either at home or in residential settings such as Family Care or Individual Residential Alternatives. Service Coordinators assist Participants in accessing needed services through the Home and Community Based Services Waiver which includes Residential Habilitation, Day Habilitation, Prevocational Services, Family Education & Training, and Supported Work Services.
The Service Coordinators work closely with local school districts, the Department of Social Services, Mental Health, Social Security and a variety of other agencies in Genesee County, for the benefit of the Individuals served. Community Services also oversees Centralized Intake for Genessee County Chapter NYSARC, Inc.
